Instructions for Completing Faculty Service Report

The Faculty Service Report assists the faculty member in identifying his/her workload in terms of each activity.  This information is used by the chairperson in completing the annual Faculty Evaluation Report. 

Material in each section should be listed in chronological order.  Sections I and II are to be completed on the provided forms.  Sections III–IV should be submitted on separate page(s).  

Section I:  Teaching Responsibilities  

List assigned classes and indicated information.  

Section II:  Academic Advising  

List assigned advisees for evaluation period.  

Section III:  Scholarly and Professional Activity

  1. Scholarly Research/Creative Projects/Grant Writing
    List papers, books, articles*, presentations at professional meeting, grants applied for and/or funded, teaching modules, lab manuals, preparation for new classes, instilling technology in existing classes, etc.

  2. Professional Organizations  
    List local, state, and national professional organizations to which you belong and offices held or membership on committees; also list service as judge, accreditation team member, etc.

  3. Maintaining Current Knowledge in Discipline  
    List attendance at professional meetings, conferences, workshops; review of articles or texts for professional publications; redesign of curricula; etc.  

Section IV:  Professional Service

  1. Institutional Service  
    List all departmental/divisional, college, and institutional committees to which you belong.  Also list activities such as organizing science fairs; conducting training workshops for faculty, staff or students, college tournaments, special recruiting days, and conferences; sponsoring a student or faculty academic society, honor society, or service organization; reading grants or establishing contracts with outside agencies.  Also list any awards or honors earned as a result of these activities.  

  2. Public Service  
    List position-related activities outside of the institution, such as consulting, public lectures, representation of the University at public functions or on municipal boards, etc.
